A 55 years old man presented to A&E with painful bilateral digital ulcers and cyanosis. He had a history of heavy smoking. His inflammatory markers were normal. A grey-scale and Doppler ultrasound (transverse and longitudinal planes) of the ulnar and radial arteries was performed, and thrombotic occlusion of the ulnar artery (white arrows) was identified.
